This actually looks like the modem might be trying to load balance across different channels to find necessary capacity to keep up with demand.  Being a non-DOCSIS 3.0 modem it has to disconnect in order to change channels.  There might be benefit to either upgrading to a DOCSIS 3.0 modem or have a technician to come out and try testing with one to see if that helps.
